Consent Form
By filling this form in I hereby declare that I give full consent to your tattooist at True Love Tattoo Studio to tattoo me. I understand that the procedure will be carried out under hygienic conditions using appropriate sterile instruments (inc. pre sterilised single use needles) and techniques.
I confirm that the information that I have provided in this consent form is true to the best of my knowledge and that I am not under the influence of alcohol or drugs. I also confirm that I have requested this tattoo at my own free will.
I confirm I am not taking any other medication or suffering from any other illness, condition or allergy which a reasonable person should be aware of, may react to, or be aggravated by tattooing. I understand that a new tattoo is susceptible to infection until healed and that proper aftercare of the tattoo once leave is my responsibility. I understand I will be given the aftercare procedure in writing and if I don’t understand it to ask.
I also confirm that I will follow the aftercare procedure until the healing process is complete. I understand that not withstanding the hygienic conditions and sterile equipment and techniques used by this tattooing establishment, there are associated risks with tattooing which include infection, scarring, allergic reactions, localised swelling. In giving this consent I release the tattoo studio, it’s employees and the self employed tattooists that operate here from all liabilities, actions and demands which i may have now or in the future from any loss or damage suffered howsoever caused as a result of my tattoo (except as a fraudulent misstatement of personal injury caused by ourselves negligence and any failure on my part to follow the aftercare procedure).
